Dementia Care Connections offers support services to family caregivers and healthcare professionals by providing education, consultations,
in-home activity visits, training, and care resources needed to improve and maintain the quality of life for individuals living with dementia. We advocate for loved ones and empower families so they can make the best choices.
How We Can Help
BASIC ASSESSMENT
1-2 HOURS
Basic Assessment includes an evaluation of cognition, level of engagement, social network, behavior observation, safety concerns, home environment, family support structure, and follow-up consultation to discuss goals of care, address areas of concern, and provide appropriate resources and referral options.
CONSULTATION
HOURLY
Consultation includes a variety of support services, but not limited to, discussing goals of care, navigating family dynamics, understanding options for care, community resources, referrals, establishing a care team, assisted living versus memory care community appropriateness, coordinating in-home services and caregiver interviews, scheduling residential community tours, accompanying family members on visits, assisting with move-in paperwork and move-in day logistics, caregiver support, strategies for managing challenging behaviors, and family education on dementia.
ACTIVITY VISIT
HOURLY & PACKAGES
Activity Visit provides cognitive engagement and socialization on a one-time or ongoing basis. Appropriate activities will be planned accordingly after learning more about their past and present interests, hobbies, music preference, favorite foods, family life growing up, career, and most cherished life experiences. We will share ideas, provide tools, and coach family members on techniques to set themselves up for success, enhance communication and engagement, and create positive "living in the moment" experiences with their loved one.
DEMENTIA TRAINING COACH
HOURLY & PACKAGES
Dementia education training for care professionals can be customized for group in-service sessions as well as one on one coaching with your team. Our services are available on an hourly basis or as a bundle option to best accommodate ongoing dementia coaching sessions. All training is tailored to meet the needs of your team.
LONG DISTANCE CAREGIVING SUPPORT
HOURLY & PACKAGES
Families who live outside of the Washington, D.C. metro or Kansas City metro but have a loved one living locally can benefit from routine visits. After each visit, our team will update and alert you as there are changes in cognition or unsafe behaviors observed. This can allow them to maintain independence and provide you with better peace of mind.
