Dementia Care Connections offers support services to family caregivers and care professionals by providing education, consultations, in-home activity visits, virtual visits, training, and care resources needed to improve and maintain the quality of life for individuals living with memory impairment and dementia. Our approach focuses on cognitive engagement, promoting social interactions, advocating for a loved one's care, and empowering families so they can make the best choices.

Nicole McMonigle Knight, Dementia Care Connections LLC

Nicole McMonigle Knight, CDP

Founder, Dementia Care Connections

Nicole has over 20 years of experience in healthcare, primarily in the field of aging. She has worked in pharmacy care, hospice care, and completed a field study in physical therapy before transitioning to dementia care in a

memory care community setting. She is a Certified Dementia Practitioner and trained in BCAT®. Nicole is a memory

cafe facilitator, support group facilitator for the Alzheimer's Association, Community Ambassador for Fairfax Area Agency on Aging 50+ program, Senior Ambassador for Senior Services of Alexandria, Dementia Champion, and on the action teams for Dementia Friendly Herndon and Dementia Friendly Alexandria. Nicole finds joy in travel, art galleries, museums, Virginia Tech football, outdoor adventures with her husband, and daily walks with her mostly well-behaved cockapoo.

Liz Anderson, Dementia Care Connections LLC

Liz Anderson, MA, CDP, R-DMT

Liz has an extensive background with over 18 years of experience facilitating creative therapy groups in a variety of settings nationwide including hospitals, inpatient psychiatric units, outpatient clinics, and private settings. She is a Certified Dementia Practitioner, Registered Dance Movement Therapist, and brings experience in individualized and group counseling, yoga therapy, dance movement therapy, creative journaling, spirituality groups, music, art, and recreation. Liz shares a passion for working with older adults, veterans, and individuals living with memory impairment. 

Aislyn Epperson, Dementia Care Connections LLC

Aislyn Epperson

Aislyn brings experience working in a variety of healthcare settings over the years with older adults and directly in a memory care community in Northern Virginia. Her experience includes planning structured in-home activities, guiding reminiscing discussions, regular client outings, and using art and music to connect with our clients. Aislyn is a co-facilitator for our memory cafe in Arlington and collaborates with outside partnering aging life care managers and home care agencies as part of a full care team approach to better serve our client's needs.

Sarah Van Winkle, Dementia Care Connections LLC

Sarah Van Winkle, CMC

Sarah has over 20 years of experience working with people with dementia. She worked for a large assisted living company for several years designing memory care programs including activities and care plans. Sarah is a Certified Care Manager and has served on the board of the Aging Life Care Association recently. She has also been trained as a support group leader through the Alzheimer’s Association and has led groups for several years. Sarah feels that working with older adults, especially those with memory impairment, is her calling and she is passionate about serving people.  

Jen Denk, Dementia Care Connections LLC

Jennifer M. Denk, MSW, CDP

Jen is a recent graduate of George Mason University where she received a Master’s Degree in Social Work and specialized in clinical practice for older adults. She is a Certified Dementia Practitioner, trained in BCAT®, and currently completing a gerontology certificate from George Mason University. Jen has worked with people experiencing all stages of dementia which included administering assessments, conducting support groups, and leading recreational activities. Actively involved in the community, she is a Dementia Champion and a member of Dementia Friends Fairfax.


Pamela is a Certified Therapeutic Recreation Specialist, Certified Alzheimer's Disease and Dementia Care Trainer, Certified Dementia Care Manager, and Assisted Living Manager. She holds a Masters of Arts degree from the Erickson School for the Management of Aging Services at University of Maryland and has over 30 years’ experience in the field of Aging Services primarily with people living with neurocognitive impairment in their homes, Memory Care, Social Day and Assisted Living settings. Her work on generating and maintaining programs that support and enhance the wellbeing for older adults has been published in Longterm Living magazine as well as presented at conferences and webinars. Most importantly, her heartfelt passion is for designing and implementing creative, insightful, person-directed services for older adults, where each person can feel comfort, joy and achievement. In addition to spending time with family, Pamela and her husband enjoy outdoor activities, volunteering, and tending to their flock of backyard fowl and household pets.
